Liability Waiver *

Peak Squash & Guyana Squash Association (GSA) – Junior Squad Liability Waiver National Racket Center I, the undersigned parent/guardian, understand and acknowledge that participation in squash and related activities involves inherent risks, including but not limited to injury from falls, collisions, or equipment use. In consideration of my child’s participation in the Peak Squash & GSA Junior Squad at the National Racket Center (NRC), I hereby: 1. Release and discharge Peak Squash, GSA , its coaches, staff, volunteers, and the NRC from any and all claims, liabilities, or demands arising from injury, loss, or damage incurred during participation. 2. Assume all risks associated with my child’s involvement in the program. 3. Consent to the provision of first aid or emergency medical care deemed necessary. 4. Grant permission for the use of my child’s name, image, or likeness for promotional purposes related to Peak Squash, the GSA and the NRC. I confirm that my child is physically fit to participate and that I have read and understood this waiver.
